Space Medicine Physicians — typically titled Flight Surgeons at NASA and ESA — are responsible for the health, safety, and performance of astronauts from initial selection through post-flight rehabilitation. The discipline encompasses microgravity physiology (cardiovascular deconditioning, bone density loss, muscle atrophy, vestibular disruption), cosmic radiation exposure modeling, behavioral health in isolation and confinement, emergency medicine with no evacuation option, and vision problems associated with Spaceflight-Associated Neuro-ocular Syndrome (SANS). As commercial crew programs (Boeing Starliner, SpaceX Crew Dragon) and the first private space stations come online, demand for space medicine expertise is growing rapidly beyond government agencies.
Any rigorous pre-med major (biology, biochemistry, physiology, kinesiology) followed by 4 years of medical school. Strong performance in physiology, pharmacology, and anatomy is critical since the spaceflight effects are essentially accelerated multi-system physiological stress. Relevant electives: aerospace physiology, exercise physiology, wilderness medicine. DO programs are now fully equal to MD programs for NASA consideration. GPA and MCAT scores remain the gate.
NASA and most space agencies require a clinical residency. The most common specialties feeding into space medicine are: Family Medicine (3 years), Internal Medicine (3 years), or Emergency Medicine (3–4 years). Military physicians often complete an Aerospace Medicine residency (USAF at Wright-Patterson AFB, US Navy at Pensacola), which combines 1 year clinical + 1 year academic (MSc Aerospace Medicine at Wright State University) and includes flight physiology and altitude chamber training. Board certification is required.
After residency, a dedicated postgraduate credential in aerospace medicine is the entry ticket. Options: Wright State University MSc Aerospace Medicine (the US standard), King's College London MSc Human Factors and Aviation Medicine (UK / ESA pipeline), MEDES Toulouse space medicine training programme (CNES), or DLR Institute of Aerospace Medicine Cologne (German/ESA pipeline). The curriculum covers altitude physiology, acceleration forces, hypoxia, radiation biology, and space-specific adaptations.
Entry-level position at NASA JSC Flight Medicine Clinic, ESA Medical Operations Cologne, or a commercial crew program medical team. Responsibilities include pre-flight medical certification exams, in-flight telemedicine consultation during ISS missions (real-time medical conferences with crew via voice and video), developing crew medical procedures, and designing countermeasures for in-flight health issues. NASA flight surgeons are assigned to specific astronaut crew assignments and remain on-call 24/7 during missions.
Lead the medical operations for a major program (e.g., Artemis lunar missions, a commercial space station medical protocol). Develop new physiological countermeasure protocols for long-duration exploration missions. Interface with human research program (HRP) researchers to translate findings into operational procedures. Some senior flight surgeons transition into human spaceflight policy roles (FAA Office of Aerospace Medicine, commercial crew medical certification), clinical research, or — most rarely — apply directly for the astronaut corps.
After 6-month ISS missions, flight surgeons manage the return-to-duty process: monitoring bone density recovery, cardiovascular reconditioning, vestibular rehabilitation, and neurocognitive assessment. For future long-duration missions (Mars transit ~9 months each way), the flight surgeon will be one of the crew — requiring combined clinical skills with operational mission roles. Research publications in this phase are expected and contribute to the human research evidence base.

The hardest part
The fundamental ethical tension: you are simultaneously the astronaut's personal physician (with patient confidentiality obligations) and the NASA program's gatekeeper who decides whether that astronaut is medically cleared to fly or must be replaced. Astronauts are selected from thousands of applicants, train for years for a mission, and you may have to remove them from a flight 6 weeks before launch due to a medical finding — and they may never fly. That conversation, and living with the downstream consequences, is something no medical school curriculum prepares you for. There is also the relentlessness of the 24/7 on-call pace during active missions and the career length required: MD + residency + fellowship before you touch a space program means your early-30s are spent still in training.
